WayStone Individualized Home Supports
At WayStone, we believe everyone deserves the opportunity to build independence while living in the comfort of their own home and community.
Through Minnesota’s 245D Waiver Program, we provide Individualized Home Supports (IHS) to help people learn and practice the skills they need for daily life. Whether you’re a client exploring services for the first time, a family looking for support, or a case manager searching for a trusted provider — WayStone is here to partner with you.
What Is Individualized Home Supports?
IHS is designed to help people gain skills for everyday living. Unlike Homemaking, which focuses on staff doing tasks for you, IHS focuses on teaching, coaching, and supporting you to do things yourself whenever possible.
With IHS, staff can help with:
- Shopping skills – making lists, budgeting, paying at the store
- Cooking and meal prep skills – planning, following recipes, kitchen safety
- Household management – cleaning routines, laundry, organization
- Daily routines – using a calendar, reminders, building healthy habits
- Money management – budgeting, paying bills, banking skills
- Community access – learning to use transportation, navigating safely
- Social and self-advocacy skills – building communication and confidence
Types of IHS Services
IHS can be provided in different ways depending on your needs:
- In the home – Support with daily routines, self-care, and home management.
- In the community – Learning skills like using transportation, shopping, or volunteering.
- With family involvement – Working together to strengthen independence while supporting caregivers.
Who Can Receive IHS?
IHS is for:
- Adults with disabilities who want to build independence
- Young adults transitioning to more independent living
- People living alone, with family, or with roommates who need skill-building support
- Families who want their loved ones to gain more confidence and self-reliance
Eligibility is determined through a MnCHOICES assessment and county case manager support plan.
